Your First Visit
On your first visit, you will need to gather your medical history and other important information for your medical records file. It is helpful if you can give us any health records that you have, such as records from previous physicians. We will also need information about your health insurance plan, for which we will need to see your insurance card, and preferred method of payment. In order to obtain all of this information from you, we ask that you arrive 15 minutes early for your first appointment.
Hospitalization We are primarily affiliated with Mount Carmel East and Grant Hospital. If you need to be hospitalized, we will visit you regularly and closely monitor your progress.
Referral to Specialists If you need to be referred to a specialist for consultation, you can be confident in knowing that he or she has been personally selected by us. When such a referral is necessary, we will remain ill close contact with the specialist to make sure that you continue to receive professional and personal care.
Office Hours and Appointments The office is open from 9:00 a.m. to 5:00 p.m. Monday through Friday. Appointments can be made by calling the office at 614-755-4200 during these hours. Please call 3 to 4 weeks ahead to schedule annual examinations.
If you must cancel an appointment, please let us know at least 24 hours in advance, so that the time may be given to another patient who needs to see us. Also, please be on time for your appointments. This will help us stay on schedule for all our patients.
Emergencies In the event of a sudden illness or other emergencies during office hours, call us at 614-755-4200, we will make every effort to see you as quickly as possible or send you to the emergency room, if needed. After hours, call our office at 614-755-4200 and your call will be taken by our 24-hour answering service, which can reach the covering physician.
Fees and Insurance Columbus Women's Care, Inc. participates as a provider for many insurance plans. H you are a member of one of these plans, your claims are filed directly with your insurance company and only co-payments, if applicable, are collected on the date of service.
If we are not a provider for your plan, payment for services is collected at the time of your visit. For your convenience we accept VISA, MasterCard, personal checks and cash. You will be given a copy of your charges with payments that can be submitted to your insurance company for reimbursement.
As a courtesy, we will bill your insurance company for any hospital or surgical procedures, with the balance being billed to the patient.
Because our office does not have a lab or x-ray capabilities, you will be billed separately for services provided by outside facilities.
If you have any questions regarding treatment, insurance, or if you need financial assistance, we will be more than happy to assist you.
Confidentiality Information in your medical records is kept in strict confidence and will not be released to any person or agency, including insurance companies and attorneys, without your written permission.
